What information is needed to set up the ClinCard?

The study team will give Greenphire your name, address, email address, telephone number, date of birth and Social Security number as part of the payment system. Your Social Security number is not required if you are only being reimbursed for study-related expenses (such as parking or food). For payments deemed to be compensation, a study team member may ask you to complete a W9 form per tax reporting requirements.
